What you will need on your first visit:

• A completed patient registration form . You can use this form also to update your details if there has been a change in phone number, address, or GP.  

• A valid GP referral to access medicare benefits

• Medicare Card

• Updated list of medications

• If you have diabetes, please bring your glucose meter or record book

We will send you out a reminder by SMS (text message) or phoen you a few days before your scheduled appointment. If for any reason you cannot attend, please notify us as soon as possible to allow us to reschedule and fill the vacancy. We reserve the right to charge a non-attendance fee for late cancellations.
